Healthy Transitions from Adolescent to Adult Workshop

The Healthy Delawareans with Disabilities (HDWD) Project at the Center for Disabilities Studies has received funding from DFRC, Inc. for "Healthy Transitions: Supporting Adolescents, Young Adults, and their Families in their Health Care Transition Process." The purpose of this program is to help adolescents and young adults with disabilities (14 to 30 years of age) and their family members take an active role and be better prepared for their transition from the pediatric to the adult health care system.

HDWD is partnering with the Family to Family Health Information Center and the Division of Transition of Care at Nemours/Alfred I. duPont Hospital for Children to offer a three-week workshop series that addresses: 1) the transition process; 2) insurance issues; and 3) how to live healthy as an adult. Sessions will be held at the Center for Disabilities Studies on November 3, 10, and 17 from 6:00 pm to 8:00 pm. A light dinner and dessert will be provided.
